ALL students involved in Athletics, Band, Cheer, and Honeybees must have a current physical on file with our school district.

• Wednesday, May 14 at CHS beginning at 8AM

• The cost is $25

Parents will need to sign their student up with a Google Form link HERE. In order for Health Point to perform the evaluation, parents must complete a registration form AND a UIL medical history form linked below. The $25 can be cash or check (to Health Point). Any forms/money that are turned in early can be collected and given to Jane Anne Giese. We do encourage them to bring it early for organizational purposes.

Physicals will take place in the High School's New Gym. There will be guides to lead the groups to and from each campus. A spreadsheet will be provided to each campus with a list of students signed up for physicals. We will begin at 8am with CIS, followed by CJH and then CHS. 

Any parent/student may choose where they get their physical, but we want to offer the opportunity and convenience so this can be completed before next school year.
